Entry form for Glammies appraisal

Please provide dates and times of performances. If you are having more than 4 performances, please enter additional dates and times in the box below.

If you have double casting, every effort will be made to arrange appraisers to see both teams. It is not always possible for the moderator to attend more than once. In the event that a moderator does not see both teams, the appraisers are still able to nominate individual performers. Whilst your production can still be considered for production awards, individual performers cannot win individual acting awards if they have not been seen by the moderator.

• I have read and accept the rules of entry for the Glammies and understand that in the event of non-availability of a full appraisal team, our production may not be eligible for full entry into the Glammies. 

• I confirm that our company is a fully paid-up member of the GDL  

• (I acknowledge that only companies who are fully paid-up members of the GDL are eligible for entry into the Glammies.) 

• If we wish to withdraw our entry, we agree to give as much notice as possible.
